Please see attached JMMC application and entry forms for download and completion. Orginal forms MUST be submitted to the JMMC offfice during office hours for processing along with the relevant fees. Please note while forms may be faxed to the office the ORIGINAL completed copy MUST be submitted to the office to complete the process i.e. issuing of membership cards and/or license. New member applicants MUST be proposed by a current JMMC member. Event entry forms can be printed and completed to be submitted to the designated event official on race days or to the designated acceptor on an event ASR.
